What to bring to the Pennsylvania permit test

Who this checklist covers: First-time standard noncommercial Class C learner permit applicants age 16 or older who are U.S. citizens, including minors and adults. Noncitizen immigration-document routes, REAL ID, transfers and road tests are outside this checklist.

Bring originals unless the official list says copies are accepted. Items marked Under 18 apply only to minors.

Application

Identity

Original proof of date of birth and identificationBring an original government-issued U.S. birth certificate with raised seal, U.S. citizenship/naturalization certificate or valid U.S. passport/book or card. Social Security number

Residency

Two proofs of residency for applicants age 18 and older18 and olderAt age 18+, bring two proofs with your name and Pennsylvania street address, such as a utility bill and lease. Health care provider completion of the medical section of DL-180DL-180 is valid for one year from the physical examination. The exam may not be more than six months before your 16th birthday.
