Key points

  • A premium is the monthly amount you pay for coverage. A deductible is what you pay for covered drugs before the plan starts paying.
  • No Medicare drug plan may have a deductible above $700 in 2027. The standard 2027 out-of-pocket threshold is $2,400 for covered Part D drugs.
  • A formulary is the plan's list of covered drugs. A pharmacy network is the list of pharmacies that work with the plan.
  • Extra Help can lower Part D premiums, deductibles, coinsurance, and other costs. People with full Medicaid, a Medicare Savings Program, or SSI get it automatically.
  • The Medicare Prescription Payment Plan spreads covered drug out-of-pocket costs across the calendar year. It does not lower the total drug cost.

The 2027 Part D numbers

Medicare.gov says no Part D plan may charge a deductible above $700 in 2027. After the deductible and other stages, your out-of-pocket spending on covered Part D drugs reaches the $2,400 threshold. Then catastrophic coverage begins and you pay $0 for covered Part D drugs for the rest of the calendar year.

These are Medicare-wide rules for standard Part D coverage. A plan's premium, formulary, pharmacy network, copayments, and coverage rules can still differ.

Formulary and pharmacy network

A formulary is a drug list. Check each medicine by exact name, strength, and form. A plan can cover a generic but not the brand, or require a coverage rule such as prior authorization or step therapy.

A pharmacy network is the group of pharmacies contracted with the plan. Ask whether your pharmacy is in network and whether the plan uses a preferred pharmacy or mail order. Do not assume a pharmacy is in network because it accepts another plan.

Extra Help

Extra Help is a Medicare program for people with limited income and resources. It helps with Part D premiums, deductibles, coinsurance, and other costs. It also removes the Part D late enrollment penalty while you receive it.

Medicare says Extra Help is automatic for people with full-benefit Medicaid, a Medicare Savings Program that pays Part B premiums, or SSI. If you do not get it automatically, apply through Social Security. We are not using 2027 income or resource limits here because the official source checked during this review lists 2026 limits, not a 2027 limit table.

Medicare Prescription Payment Plan

This voluntary payment option works with a Medicare drug plan or a Medicare Advantage plan with drug coverage. Instead of paying the pharmacy for covered drugs, you receive a bill from the plan and spread the year's out-of-pocket costs across the calendar year.

It does not save money or lower the drug cost. Ask the plan for the monthly bill calculation before joining, especially if the first few months of the year have large prescriptions.

Use Plan Finder

Open Medicare Plan Finder, choose 2027, enter the ZIP code, and add every prescription. Use the exact drug name and dosage. Compare the formulary, pharmacy network, premium, deductible, estimated yearly drug cost, and plan rules.

After Plan Finder shows a plan, open the plan details and call the plan. Ask the pharmacist and doctor's office to confirm the plan information for 2027. Keep a screenshot or printout of what you checked.

Steps

  1. Make a medicine list. Include name, strength, dosage, and how often you take it.
  2. Add your pharmacy. Check the plan's 2027 network and whether it has a preferred or mail-order option.
  3. Check Extra Help. Look for automatic eligibility or apply through Social Security. Do not use an unverified 2027 limit.
  4. Compare the complete year. Review premium, deductible, covered drugs, pharmacy, rules, and estimated yearly cost.

Quick check

  • I used the 2027 Plan Finder setting.
  • Every drug is on the formulary or I know the exception process.
  • My pharmacy is in the plan's 2027 network.
  • I know whether I get Extra Help.
  • I understand the difference between paying less and spreading payments.

Questions to ask

  • Is this exact drug and strength covered in 2027?
  • Does the drug need prior authorization, step therapy, or quantity limits?
  • Is my pharmacy in network and preferred?
  • Would Extra Help or the Prescription Payment Plan apply to me?

Common questions

No Medicare drug plan may have a deductible above $700 in 2027. Some plans may have a lower deductible.

What is the 2027 Part D out-of-pocket threshold?

The standard 2027 threshold is $2,400 for covered Part D drugs. After reaching it, catastrophic coverage begins and you pay $0 for covered Part D drugs for the rest of the calendar year.

What is a formulary?

A formulary is a plan's list of covered prescription drugs. Check it before joining and again when the plan year changes.

You do not pay QualitySpace for Medicare help. If you enroll in a Medicare Advantage or Part D plan through us, that plan may pay our agency a commission, within limits Medicare sets for those plans. Your premium is the same whether you enroll through us, directly with the plan, or through Medicare.gov.
